Practiva Health offers health education on behalf of physicians.
Our mission is to help patients identify their health risk factors and educate them on how to make healthy lifestyle changes.
We assist patients in preventing and/or managing chronic diseases and conditions, in order to achieve and maintain an optimal quality of life and reduce costs associated with these diseases.

People with multiple high risk factors need more than information, they need accountability, goal setting and most importantly, support.

Practiva Health offers this guidance from highly trained health professionals such as dietitians, exercise physiologists, nurses, health educators and diabetes educators.
|
|
This support is offered via scheduled telephonic appointments with an assigned health coach.
|
|
|
All information and discussion remains private between the patient and their health coach;
no individual information goes back to your employer or health plan!
The referring medical provider of course has access to all patient-data and works together with the health coach to ensure that the the Medical Wellness objectives are achieved.
